Angiotensin II receptor antagonist

Therapeutic Actions
Selectively blocks the binding of angiotensin II to specific tissue receptors found in the vascular smooth muscle and adrenal gland; this action blocks the vasoconstriction effect of the renin-angiotensin system, as well as the release of aldosterone, leading to decreased BP.
Indications
Treatment of hypertension, alone or in combination with other antihypertensive
CV risk reduction, including risk of MI, stroke, or death, in patients 55 yr and older at high risk for developing major CV events who are unable to take ACE inhibitors
Contraindications and Cautions
Contraindicated with hypersensitivity to telmisartan, pregnancy (use during the second or third trimester can cause injury or death to the fetus), lactation.
Use cautiously with hepatic or biliary impairment, hypovolemia.
Available Forms
Tablets—20, 40, 80 mg
Dosages
Adults
Usual starting dose, 40 mg PO daily. Adjust dosage based on patient response. Range, 20–80 mg/day. If response is still not as expected, a diuretic may be added. For CV risk reduction, 80 mg PO once a day.
Pediatric patients
Safety and efficacy not established.
