Standards of Practice





STANDARD 1. ASSESSMENT

The gerontological nurse collects comprehensive data pertinent to the older adult’s physical and mental health or situation.


Measurement Criteria

The gerontological nurse:



  • Prioritizes data collection activities based on the older adult’s immediate condition or needs, personal goals for physical and mental health care, cultural beliefs, socioeconomic factors, and anticipated needs;


  • Collects data in a systematic and ongoing process using appropriate evidence-based, standardized assessment tools and techniques;


  • Involves the older adult, family, significant others, designated caregiver, and other healthcare providers, across environments of care, as appropriate, in holistic data collection;


  • Uses analytical models and problem-solving tools; and


  • Documents relevant data, including risk factors and medications, in a retrievable format.


Additional Measurement Criteria for the Gerontological Advanced Practice Registered Nurse:

The gerontological advanced practice registered nurse:



  • Initiates and interprets diagnostic tests and procedures relevant to the older adult current status; and


  • Completes comprehensive assessments that identify the older adult’s specialized needs.



STANDARD 2. DIAGNOSIS

The gerontological nurse analyzes the assessment data to determine the diagnoses or issues.


Measurement Criteria

The gerontological nurse:



  • Derives the diagnoses, needs, and issues based on assessment data;


  • Validates the diagnoses, needs, and issues with the older adult, family, significant others, designated caregiver, and other healthcare providers; and


  • Documents diagnoses, needs, and issues in a manner that facilitates the determination of the expected outcomes and plan.


Additional Measurement Criteria for the Gerontological Advanced Practice Registered Nurse

The gerontological advanced practice registered nurse:



  • Systematically compares and contrasts clinical findings with normal and abnormal variations, including co-occurring psychiatric symptoms and developmental events, in formulating a differential diagnosis;


  • Uses complex data and information obtained during interview, examination, and diagnostic procedures in identifying diagnoses; and


  • Assists staff in developing and maintaining competence in the diagnostic process related to the older adult.



STANDARD 3. OUTCOMES IDENTIFICATION

The gerontological nurse identifies expected outcomes for a plan individualized to the older adult or situation.


Measurement Criteria

The gerontological nurse:



  • Involves the older adult, family, significant others, designated caregiver, and other healthcare providers in formulating expected outcomes when possible and appropriate;


  • Derives culturally appropriate expected outcomes from the diagnoses;


  • Considers current scientific evidence, clinical expertise, associated risks, benefits, and costs when formulating expected outcomes;


  • Defines expected outcomes in terms of the older adult, older adult’s values, ethical considerations, environment, and situation with consideration to current scientific evidence, associated risks, benefits, and costs;


  • Includes a time estimate for attainment of expected outcomes;


  • Develops expected outcomes that provide direction for continuity of care;


  • Modifies expected outcomes based on changes in the status of the older adult or evaluation of the situation; and


  • Documents expected outcomes as measurable goals.


Additional Measurement Criteria for the Gerontological Advanced Practice Registered Nurse:

The gerontological advanced practice registered nurse:



  • Identifies expected outcomes that incorporate scientific evidence and are achievable through implementation of evidence-based practice initiatives;


  • Identifies expected outcomes that incorporate clinical effectiveness and cost, older adult satisfaction, and continuity and consistency among providers; and


  • Supports the use of clinical guidelines linked to positive outcomes for older adults.
