Therapeutic Actions
Bactericidal: Inhibits synthesis of cell wall of sensitive organisms, causing cell death.
Indications
Treatment of severe infections caused by sensitive organisms—streptococci, pneumococci, staphylococci, Neisseria gonorrhoeae, Treponema pallidum, meningococci, Actinomyces israelii, Clostridium perfringens and tetani, Leptotrichia buccalis (Vincent disease), Spirillum minus or Streptobacillus moniliformis, Listeria monocytogenes, Fusobacterium fusiformisans, Pasteurella multocida, Erysipelothrix insidiosa, Escherichia coli, Enterobacter aerogenes, Alcaligenes faecalis, Salmonella, Shigella, Proteus mirabilis, Corynebacterium diphtheriae, Bacillus anthracis
Treatment of syphilis, gonococcal infections
Unlabeled use: Treatment of Lyme disease
Contraindications and Cautions
Contraindicated with allergy to penicillins, cephalosporins, imipenem, beta-lactamase inhibitors, other allergens.
Use cautiously with renal disease, pregnancy, lactation (may cause diarrhea or candidiasis in the infant).

Dosages
Adults
Meningococcal meningitis: 1–2 million units every 2 hr IM or by continuous IV infusion of 20–30 million units/day.
Actinomycosis: 1–6 million units/day IM in divided doses every 4–6 hr for 6 wk or IV for cervicofacial cases; 10–20 million units/day IV for thoracic and abdominal diseases.
Clostridial infections: 20 million units/day in divided doses every 4–6 hr IM or IV with antitoxin therapy.
Fusospirochetal infections (Vincent disease): 5–10 million units/day IM or IV in divided doses every 4–6 hr.
Rat-bite fever: 12–20 million units/day IM or IV in divided doses every 4–6 hr for 3–4 wk.
Listeria infections: 15–20 million units/day IM or IV in divided doses every 4–6 hr for 2 or 4 wk (meningitis or endocarditis, respectively).
Pasteurella infections: 4–6 million units/day IM or IV in divided doses every 4–6 hr for 2 wk.
Erysipeloid endocarditis: 12–20 million units/day IM or IV in divided doses every 4–6 hr for 4–6 wk.
