Therapeutic Actions
Bactericidal: Inhibits cell wall synthesis of sensitive organisms, causing cell death.
Indications
Infections due to penicillinase-producing staphylococci; may be used to initiate treatment when a staphylococci infection is suspected
Unlabeled use: Catheter-related bloodstream infections
Contraindications and Cautions
Contraindicated with allergies to penicillins, cephalosporins, or other allergens.
Use cautiously with renal disorders, pregnancy, lactation (may cause diarrhea or candidiasis in infants).

Powder for injection—500 mg; 1, 2 g
Dosages
Maximum recommended dosage is 6 g/day.
Adults and pediatric patients weighing 40 kg or more
250–500 mg every 4–6 hr IV. Up to 1 g every 4–6 hr in severe infections.
Pediatric patients weighing less than 40 kg
Neonates weighing less than 2 kg: 25–50 mg/kg every 12 hr IV.
Neonates weighing 2 kg or more: 25–50 mg/kg every 8 hr IV.
Children weighing less than 40 kg: 50–100 mg/kg/day IV in equally divided doses every 4–6 hr.
