Drug Classes

Antihypertensive

Sympatholytic (centrally acting)


Therapeutic Actions

Stimulates central (CNS) alpha2-adrenergic receptors, reduces sympathetic nerve impulses from the vasomotor center to the heart and blood vessels, decreases peripheral vascular resistance, and lowers systemic BP.


Indications



  • Management of hypertension, alone or with a thiazide diuretic


  • Treatment of ADHD as monotherapy or as adjunctive therapy with stimulants (ER tablets)


  • Unlabeled uses: Amelioration of symptoms in heroin withdrawal; reduced frequency of migraine headaches and reduced nausea and vomiting; Tourette syndrome

Dosages

Adults and children 12 yr and older



  • Hypertension: Recommended dose is 1 mg/day PO given at bedtime to minimize somnolence. If 1 mg/day does not give a satisfactory result after 3–4 wk of therapy, doses of 2 mg and then 3 mg may be given, although most of the drug’s effect is seen at 1 mg. If BP rises toward the end of the dosing interval, divided dosage should be used. Higher daily doses (rarely up to 4 mg/day in divided doses) have been used, but adverse reactions increase with doses larger than 3 mg/day, and there is no evidence of increased efficacy. Taper ER form when discontinuing; decrease by no more than 1 mg every 3–7 days.
