I have informed Nurse Alan Jones of Sam’s reasons for attendance to ED today. I have informed him that Sam is with mum, currently settled and has been assessed and treated for minor self-harm wounds to her wrist. Medically she is fit for discharge however she requires a CAMHS assessment. Alan reports staff are currently finishing morning handover and a member of the team will be attending the hospital to assess Sam shortly afterwards. Whilst an exact time cannot be given as there are other patients also awaiting assessment in hospital, CAMHS will attend ED when they arrive to provide an updated estimated time of assessment.

I have informed Sally Mills from children’s social care of Sam’s reasons for attendance to ED. Sally will inform Sam’s social worker this morning. Nurses are to contact Sam’s social worker again once CAMHS have assessed her to inform social care of Sam’s care plan and ensure follow up care is arranged.
