A healthcare prescriber’s order is required to discharge a child or transfer a child to a different level of care.
Prior to implementation of discharge/transfer processes, the child will be identified utilizing two different patient identifiers (see Chapter 82) and verification of the identification of parents or legal guardians will be completed following institutional policy (see Chapter 118).
A minor must be discharged to his or her Parent/legal guardian, unless other arrangements have been made and are documented in the child’s medical record.
Written parental/legal guardian informed consent should be obtained to transfer the child to another facility.
Transfer to another facility is treated as a discharge.
The healthcare prescriber is notified immediately if a parent/legal guardian wishes to take the child from the hospital against medical advice (AMA).
To ensure the child’s safety during transport, use the mode of transport most appropriate to the child’s age, developmental level, and condition.
All written instructions for the family are in a language, reading level, and format that the parent verifies he or she can understand.
For the child with ongoing or unresolved medical concerns, identify at least two family caregivers that will participate in discharge planning, teaching, and provision of care in the home/community.
