Somewhere in most medication reconciliations there is a moment where a patient reaches into a bag and produces a bottle. Sometimes it is a supermarket own-brand with no lot code, no manufacturer contact, and nothing you can look up. Sometimes it is a Joy Organics tincture with a batch number printed on the carton that pulls up a full third-party panel in about ten seconds. Those two conversations go completely differently, and the thing that separates them is the certificate of analysis.
Most nurses were never taught to read one. It is not in the curriculum, it rarely comes up in orientation, and yet it is one of the more practical skills available for the supplement conversations that happen constantly at the bedside and in clinic.
Why This Falls to Nursing
Dietary supplements in the United States are regulated under a framework that treats them more like food than like drugs. They do not undergo premarket review for safety or efficacy, and manufacturers carry the responsibility for the accuracy of their own labels. Good manufacturing practice requirements exist and are enforced, but the burden of verification largely sits downstream.
Meanwhile, a large share of adults take at least one supplement, and rates are higher still in older patients, who also tend to be the ones on the most medications. Disclosure is inconsistent, partly because patients do not think of supplements as drugs and partly because they anticipate being dismissed for using them.
Nurses are frequently the person in the room when the bottle comes out. Being able to open the manufacturer’s testing document and say something specific about it changes that interaction entirely.
What a Certificate of Analysis Is
A certificate of analysis is a laboratory report on a specific production batch. It states what was tested, what method was used, what was found, and whether the result met the specification. For a botanical or cannabinoid product it should be produced by an outside laboratory rather than the manufacturer’s own bench.
The single most useful marker of a credible report is accreditation to ISO/IEC 17025, which is the international standard for testing laboratory competence. It will usually appear in the letterhead or footer. A report from an accredited lab is not a guarantee of a good product, but a report from an unaccredited one tells you very little.
Step One: Match the Batch
Before reading any results, confirm that the lot or batch number on the report matches the number printed on the bottle in front of you. This is the step people skip and it is the one that matters most, because a report for a different batch is simply not evidence about the product the patient is taking.
Check the date as well. A certificate should be roughly contemporaneous with the production run, and a report several years older than the product on the counter is a mismatch worth noticing. Many manufacturers now print a QR code or a batch lookup field that pulls the correct document directly, which removes the guesswork.
Step Two: Read the Potency Panel
The potency or cannabinoid panel tells you what is actually in the product, and the units are where clinicians most often get tripped up. Results may be expressed as a percentage by weight, as milligrams per gram, or as milligrams per serving, and these are not interchangeable. What you want for documentation is milligrams per serving and the serving size the patient actually uses.
For hemp products there is a small technical detail worth knowing. Raw plant material contains cannabinoids in acid form, written as CBDA and THCA, which convert to CBD and THC when heated. A well-constructed report gives both the acid and neutral forms and a calculated total that accounts for the conversion. If a report lists only one form without explanation, the number may not mean what it appears to mean.
The same applies to THC content. Hemp is defined federally by a threshold of 0.3 percent delta-9 THC by dry weight, but a percentage tells your patient nothing useful about whether a serving could affect a workplace drug screen. Milligrams per serving does.
Step Three: Read the Contaminant Panels
A complete report covers four contaminant categories, and the absence of any of them is informative in itself.
Heavy metals testing should cover lead, arsenic, cadmium, and mercury. This panel matters more for hemp than for most botanicals because the plant readily takes up metals from soil, to the point that it has been studied for use in soil remediation. Growing conditions therefore have a direct bearing on what ends up in the bottle.
Pesticide screening panels vary in breadth depending on the state and the standard applied, and a broader panel is better. Residual solvent testing reflects the extraction method, since solvent-based extraction can leave traces of things like ethanol, butane, or propane, while carbon dioxide extraction largely avoids the question. Microbial testing should cover total aerobic count, yeast and mould, and specific pathogens including Salmonella and E. coli, and mycotoxin screening for aflatoxins and ochratoxin A is a sign of a thorough programme.
Results are commonly reported as ND, meaning not detected, alongside a limit of detection or limit of quantitation. ND does not mean zero. It means below the threshold the instrument could measure, which is why the stated limit is part of the result rather than a footnote.
What the Certifications on the Front Actually Mean
Front-of-package marks vary enormously in what they are worth, and it helps to know which ones involve an outside party.
USDA organic certification is among the more substantive, because it requires inspection by an accredited certifying agent against a defined federal standard covering agricultural inputs, soil, and handling. It is comparatively rare on hemp extracts, since the certification has to hold across the whole supply chain rather than just the farm. Joy Organics carries it across its tincture line and publishes a batch-specific panel for every lot it sells, which together is close to the ceiling of what a product in this category can currently document.
Certification and laboratory testing answer different questions, and it is useful to keep them separate in your head. Organic certification speaks to how something was grown and processed. The certificate of analysis speaks to what ended up in the finished bottle. A thorough manufacturer provides both.
Other marks worth recognising include NSF and USP verification, which involve independent testing and facility review, and GMP registration, which speaks to manufacturing process rather than to the contents of any particular batch.
Signals That Warrant a Closer Look
A few patterns should slow you down. A report with no batch number, or one that does not match the bottle. A laboratory with no accreditation listed. Results reported only as pass or fail with no numerical values. Missing contaminant panels. A measured potency substantially different from what the label claims. And a product for which no report can be located at all, which is the clearest signal of the group.
None of these tell you a product is unsafe. They tell you that you cannot verify it, which is a different statement and one worth conveying to the patient in exactly those terms.
The Interaction Question
Quality assessment answers what is in the bottle. It does not answer whether the patient should be taking it alongside their current regimen, and for cannabinoid products that second question deserves attention.
CBD inhibits several cytochrome P450 enzymes, notably CYP3A4 and CYP2C19, which metabolise a substantial share of commonly prescribed drugs. The clinically documented interactions include clobazam, valproate, and warfarin, and the general principle extends to any agent with a narrow therapeutic window. Once you have established an accurate milligram dose from the certificate of analysis, that figure is what makes a meaningful pharmacy consult possible.
What to Document
Record the product name and manufacturer, the batch number, the actual dose in milligrams per serving, the serving size, the frequency, and the route. A note reading that the patient takes CBD oil is close to useless. A note reading that the patient takes 25 milligrams of CBD orally each evening from a specified product and batch is something a pharmacist can act on.
Where a certificate of analysis is available, noting that it was reviewed and that the batch matched takes a few extra words and tells the next clinician that the dose figure is reliable rather than estimated.
Making It Fit in the Time You Have
None of this needs to become a project. In practice it collapses to a short sequence: ask what they are taking and how much, ask whether they can pull up the test report for that batch, check that the lot numbers match, read the milligrams per serving, glance at whether the contaminant panels are present, and document the number.
It takes a couple of minutes and it accomplishes two things at once. It gives you a defensible figure for the record, and it signals to the patient that their supplement use is something you take seriously enough to examine properly. That second effect is worth more than it sounds, because patients who feel their choices will be engaged with rather than dismissed are considerably more likely to tell you about the next one.
