Antiarrhythmic
Therapeutic Actions
Selectively blocks potassium channels, widening the QRS complex and prolonging the action potential; has no effect on calcium channels or cardiac contraction; Class III antiarrhythmic.
Indications
Conversion of atrial fibrillation or flutter to normal sinus rhythm
Maintenance of normal sinus rhythm in patients with atrial fibrillation or flutter of more than 1-week’s duration who have been converted to sinus rhythm
Contraindications and Cautions
Contraindicated with hypersensitivity to dofetilide, ibutilide; second- or third-degree AV heart block; prolonged QT intervals; lactation.
Use cautiously with ventricular arrhythmias, renal impairment, severe hepatic impairment, pregnancy.

Capsules—125, 250, 500 mcg
Dosages
Adults
Dosage is based on ECG response and CrCl. For CrCl greater than 60 mL/min, 500 mcg bid PO; for CrCl 40–60 mL/min, 250 mcg bid PO; for CrCl 20–less than 40 mL/min, 125 mcg bid PO; for CrCl less than 20 mL/min, use is contraindicated.
Pediatric patients
Not recommended.
