Antiarrhythmic


Therapeutic Actions

Selectively blocks potassium channels, widening the QRS complex and prolonging the action potential; has no effect on calcium channels or cardiac contraction; Class III antiarrhythmic.


Indications



  • Conversion of atrial fibrillation or flutter to normal sinus rhythm


  • Maintenance of normal sinus rhythm in patients with atrial fibrillation or flutter of more than 1-week’s duration who have been converted to sinus rhythm

Capsules—125, 250, 500 mcg


Dosages

Adults

Dosage is based on ECG response and CrCl. For CrCl greater than 60 mL/min, 500 mcg bid PO; for CrCl 40–60 mL/min, 250 mcg bid PO; for CrCl 20–less than 40 mL/min, 125 mcg bid PO; for CrCl less than 20 mL/min, use is contraindicated.

Pediatric patients

Not recommended.
