Delivering palliative approaches in different care contexts Most patients who can express a choice initially want to be cared for and die at home, but many later change their mind. Care in the last few days of life is possible at home, but careful planning and support is needed, preferably starting before the patient approaches their final days (Chapters 54 and 55). The Preferred Place of Care document and the Gold Standards Framework can help facilitate this planning (Chapter 6). Most people with a life-threatening illness remain at home for the majority of their illness and their families may need help and support in caring for them. Where palliative care is available, it is most commonly provided by community nurses visiting people in their homes. There are different models of nursing in the community seen in different places and according to the resources available. The following are examples of models that may be seen:
