Therapeutic Actions
Indications
Relief of moderate to severe pain when use of an opioid analgesic is appropriate
For preoperative or preanesthetic medication, to supplement balanced anesthesia, and to relieve prepartum pain (parenteral)
Nasal spray: Relief of moderate to severe pain
Contraindications and Cautions
Contraindicated with hypersensitivity to butorphanol or benzethonium chloride preservative, physical dependence on an opioid analgesic, pregnancy, lactation.
Use cautiously with bronchial asthma, COPD, respiratory depression, anoxia, increased intracranial pressure, acute MI, ventricular failure, coronary insufficiency, hypertension, biliary tract surgery, renal or hepatic impairment, geriatric patients.

Dosages
Adults
IM
Usual single dose is 2 mg every 3–4 hr; patient must be able to remain recumbent. Dosage range is 1–4 mg every 3–4 hr; single doses should not exceed 4 mg.
Preoperative: 2 mg IM, 60–90 min before surgery.
IV
Usual single dose is 1 mg every 3–4 hr. Dosage range is 0.5–2 mg every 3–4 hr.
