Therapeutic Actions
Exact mechanism of action not understood; acts with glutamate and GABA neurotransmitter systems in the CNS to restore balance between neuronal excitation and inhibition that may be altered by chronic alcohol exposure.
Indications
Contraindications and Cautions
Contraindicated with allergy to any component of the drug or severe renal impairment (CrCl 30 mL/min or less).
Use cautiously with pregnancy, lactation, moderate renal impairment, history of depression and suicidal thoughts.

DR tablets—333 mg
Dosages
Adults
666 mg (two 333-mg tablets) PO tid; may be taken with meals to aid compliance. Dosage should begin as soon as possible after period of alcohol withdrawal when the patient has achieved abstinence, and should be maintained if patient relapses. Should be part of a comprehensive psychosocial treatment program.
Pediatric patients
Safety and efficacy not established.
Patients with renal impairment
Moderate impairment (CrCl 30–50 mL/min): 333 mg PO tid; severe renal impairment (CrCl 30 mL/min or less): do not use acamprosate.
