Therapeutic Actions
Mechanism of action not fully understood; blocks dopamine and serotonin receptors in the brain, depresses the reticular activating system; effective in suppressing many of the negative aspects of schizophrenia (blunted affect, social withdrawal, lack of motivation, anger).
Indications
Treatment of schizophrenia and to delay the time and rate of relapse
Oral: Treatment of acute bipolar mania including manic or mixed episodes associated with bipolar disorder with or without psychotic features; maintenance treatment, used with lithium and valproate
IM: Rapid control of agitated behavior and psychotic symptoms in patients with acute schizophrenia exacerbations
Unlabeled uses: Autism, Tourette syndrome
Contraindications and Cautions
Contraindicated with allergy to ziprasidone, prolonged QTc interval, history of severe cardiac disease, dementia-related psychosis.
Use cautiously with renal or hepatic impairment, CV disease, history of seizures, history of drug abuse/dependence, pregnancy, lactation.

Capsules—20, 40, 60, 80 mg; powder for injection—20 mg/mL
Dosages
Adults
Schizophrenia: Initially, 20 mg PO bid with food. Adjust as needed. Effective range, 20–100 mg PO bid.
Rapid control of agitated behavior: 10–20 mg IM; doses of 10 mg may be repeated every 2 hr; doses of 20 mg may be repeated in 4 hr. Maximum dose, 40 mg/day.
