The vancomycins currently have only one member of the group, which is the prototype vancomycin (Vancocin). This drug is a naturally occurring antibiotic and it is bactericidal. It is used in the treatment of severe infections, but because it produces very severe toxic effects, its use is limited. Initially, vancomycin (Vancocin) had very widespread use in the treatment of Staphylococcus aureus infections. The Centers for Disease Control and Prevention have recommended a decreased use of vancomycin (Vancocin) to limit the spread of vancomycin-resistant organisms. Vancomycin (Vancocin) is usually given intravenously as the absorption from the gastrointestinal (GI) tract is unsatisfactory. The oral route is used for some GI infections. It is distributed into almost all body fluids and tissues and has a serum half-life of 4 to 6 hours in clients with normal kidney function. In those with impaired kidney function and in the elderly, the half-life can last up to 146 hours. The intravenous form of the drug is excreted mainly through the kidney, while the oral form is excreted in the feces.

Take at evenly spaced intervals around the clock to keep blood levels even.
Report hearing abnormalities to physician immediately.
Advise physician of skin rash, fever, or sore throat.
Report for ordered blood work: complete blood count, (CBC), peak, and trough levels.
Develop awareness of I & O.
