Therapeutic Actions
Competitively blocks muscarinic receptor sites; bladder contraction is mediated by muscarinic receptors—blocking these receptors decreases bladder contraction, relieving bladder spasm.
Indications
Treatment of urinary incontinence, urgency, and frequency caused by overactive bladder
Contraindications and Cautions
Contraindicated with allergy to trospium or any component of the drug; presence of or risk for urinary retention, gastric retention, uncontrolled narrow-angle glaucoma.
Use cautiously with renal or hepatic impairment, ulcerative colitis, intestinal atony, myasthenia gravis, bladder or gastric outlet obstruction, pregnancy, lactation.
Available Forms
Tablets—20 mg; ER capsulesDNC—60 mg
Dosages
Adults younger than 75 yr
20 mg PO bid taken on an empty stomach, at least 1 hr before meals. Alternatively, 60 mg ER tablet PO once/day.
Adults 75 yr and older
Monitor patient response and adjust dosage down to 20 mg/day PO.
Pediatric patients
Safety and efficacy not established.
Patients with renal impairment
For CrCl less than 30 mL/min, 20 mg/day PO given at bedtime. ER tablets are not recommended.
