Therapeutic Actions
Anti-inflammatory, analgesic, and antipyretic activities largely related to inhibition or prostaglandin synthesis; exact mechanisms of action are not known.
Indications
Treatment of acute flares and long-term management of rheumatoid arthritis and osteoarthritis
Treatment of juvenile rheumatoid arthritis in patients older than 2 yr
Contraindications and Cautions
Contraindicated with hypersensitivity to the drug or any of its components, in patients who have had reactions to aspirin or other NSAIDs; with lactation, treatment of perioperative pain in the setting of CABG surgery.
Use cautiously with allergies; renal, hepatic, CV and GI conditions; coagulation defects; patients taking anticoagulants; pregnancy.
Available Forms
Tablets—200, 600 mg; capsules—400 mg
Dosages
Adults
Do not exceed 1,800 mg/day. Doses exceeding 1.8 g daily have not been studied. Subsequent dosing adjustment based on response and tolerance after 1–2 wk.
Rheumatoid arthritis or osteoarthritis: Initial dose, 400 mg PO tid (1,200 mg/day) preferably including dose on arising and at bedtime. Maintenance dose, 600–1,800 mg/day in three to four divided doses for rheumatoid arthritis; 600–1,600 mg/day in three to four divided doses for osteoarthritis.
Pediatric patients 2 yr and older
Initially, 20 mg/kg/day PO in three to four divided doses; when control has been achieved, the usual dose is 15–30 mg/kg/day. Do not exceed 30 mg/kg/day.
