Beta-adrenergic blocker (nonselective)
Therapeutic Actions
Competitively blocks beta-adrenergic receptors but also has some intrinsic sympathomimetic activity; however, the mechanism by which it lowers BP is unclear, because it only slightly decreases resting cardiac output and inconsistently affects plasma renin levels.
Indications
Management of hypertension, alone or with other drugs, especially diuretics
Unlabeled uses: Treatment of fibromyalgia; prevention of migraine; traumatic brain injury
Contraindications and Cautions
Contraindicated with sinus bradycardia, second- or third-degree heart block, cardiogenic shock, HF, bronchial asthma, severe COPD, pregnancy (embryotoxic in preclinical studies), lactation.
Use cautiously with diabetes, thyrotoxicosis, hepatic impairment.

Tablets—5, 10 mg
Dosages
Adults
Initially, 5 mg PO bid. Adjust dosage as needed in increments of 10 mg/day at 3- to 4-wk intervals to a maximum of 60 mg/day. Usual maintenance dose is 5 mg tid.
Pediatric patients
Safety and efficacy not established.
