Therapeutic Actions

Gastric acid-pump inhibitor: Suppresses gastric acid secretion by specific inhibition of the hydrogen-potassium ATPase enzyme system at the secretory surface of the gastric parietal cells; blocks the final step of acid production.


Indications



  • Oral: Short-term (8 wk or less) and long-term treatment of GERD


  • Maintenance healing of erosive esophagitis


  • Long-term treatment of pathologic hypersecretory conditions


  • IV: Short-term (7–10 days) treatment of GERD in patients unable to continue oral therapy


  • IV: Treatment of pathologic hypersecretory conditions associated with Zollinger-Ellison syndrome and other neoplastic conditions


  • Unlabeled uses: Treatment of duodenal ulcer, prevention of GI bleeding in patients receiving antiplatelet drugs, laryngitis, prophylaxis for stress ulcer

Dosages

Adults

40 mg PO daily for short-term treatment and maintenance healing of erosive esophagitis for 8 wk or less. The 8-wk course may be repeated if healing has not occurred. Give continually for hypersecretory disorders: 40 mg/day IV bid, up to 240 mg/day (2-yr duration) or 40 mg/day IV for 7–10 days. For severe hypersecretory syndromes, 40 mg every 12 hr (up to 240 mg/day) PO or IV (6-day duration) has been used.

Pediatric patients 5 yr and older



  • Short-term treatment of GERD: 15 kg to less than 40 kg: 20 mg/day PO for up to 8 wk. 40 kg or more: 40 mg/day PO for up to 8 wk.

Patients with hepatic impairment
