Therapeutic Actions
Bactericidal; interferes with DNA replication, repair, transcription, and recombination in susceptible gram-negative and gram-positive bacteria, preventing cell reproduction and leading to cell death.
Indications

Treatment of adults with community-acquired pneumonia caused by susceptible strains of Streptococcus pneumoniae (including multidrug resistant strains), Haemophilus influenzae, Mycoplasma pneumoniae, Chlamydia pneumoniae, Moraxella catarrhalis, Staphylococcus aureus, Klebsiella pneumoniae

Treatment of bacterial sinusitis caused by S. pneumoniae, H. influenzae, M. catarrhalis

Treatment of acute bacterial exacerbation of chronic bronchitis caused by S. pneumoniae, H. influenzae, H. parainfluenzae, K. pneumoniae, S. aureus, M. catarrhalis

Treatment of uncomplicated skin and skin structure infections caused by S. aureus or Streptococcus pyogenes

Treatment of complicated skin and skin structure infections and complicated intra-abdominal infections caused by methicillin–susceptible S. aureus, E. coli, K. pneumoniae, Enterobacter cloacae

Treatment of bacterial conjunctivitis caused by susceptible strains of bacteria (ophthalmic solution)

Unlabeled use: Hospital-acquired pneumonia, infective endocarditis in adults
Contraindications and Cautions

Contraindicated with allergy to fluoroquinolones, prolonged QT interval, hypokalemia, pregnancy, lactation, history of myasthenia gravis.

Use cautiously with hepatic impairment, seizures.

Tablets—400 mg; injection—400 mg in 250 mL; ophthalmic solution—0.5%
Dosages
Adults
-
Pneumonia: 400 mg PO or IV daily for 7–14 days.

Sinusitis: 400 mg PO or IV daily for 10 days.

Acute exacerbation of chronic bronchitis: 400 mg PO or IV daily for 5 days.

Uncomplicated skin and skin structure infections: 400 mg PO daily for 7 days.
-
Complicated skin and skin structure infections: 400 mg PO or IV daily for 7–21 days.
-
Complicated intra-abdominal infections: 400 mg PO or IV for 5–14 days.

Bacterial conjuncitivitis: 1 drop in affected eye(s) bid for 7 days (Moxeza) or tid for 7 days (Vigamox).
