Therapeutic Actions
Hydrolyzed in acid urine to ammonia and formaldehyde, which is bactericidal; the hippurate and mandelate salts help to maintain an acid urine.
Indications
Suppression or elimination of bacteriuria associated with pyelonephritis, cystitis, chronic UTIs, residual urine (accompanying some neurologic disorders), and in anatomic abnormalities of the urinary tract
Contraindications and Cautions
Contraindicated with allergy to methenamine, tartrazine (in methenamine hippurate marketed as Hiprex), aspirin (associated with tartrazine allergy), lactation, severe dehydration, renal or hepatic impairement.
Use cautiously with gout (causes urate crystals to precipitate in urine), pregnancy.

1 g four times a day PO after meals and at bedtime.
