Therapeutic Actions
Inhibits reabsorption of sodium and chloride in distal renal tubule, increasing excretion of sodium, chloride, and water by the kidneys; may decrease peripheral resistance.
Indications
Edema associated with heart failure
Hypertension, as sole therapy or in combination with other antihypertensives
Unlabeled use: Diabetes insipidus, especially nephrogenic diabetes insipidus
Contraindications and Cautions
Contraindicated with allergy to thiazides, sulfonamides; hepatic coma or precoma, anuria.
Use cautiously with fluid or electrolyte imbalance; renal disease (risk of azotemia); liver disease (may precipitate hepatic coma); gout (risk of precipitation of attack); SLE; glucose tolerance abnormalities, diabetes mellitus; hyperparathyroidism; bipolar disorder (aggravated by hypercalcemia); pregnancy; lactation.

Tablets—1.25, 2.5 mg
Dosages
Adults
Edema: 2.5 mg/day PO as single dose in the morning. May be increased to 5 mg/day if response is not satisfactory after 1 wk.
Hypertension: 1.25 mg/day PO. May be increased up to 2.5 mg/day if response is not satisfactory after 4 wk. May increase to a maximum of 5 mg/day or consider adding another antihypertensive. If combination antihypertensive therapy is needed, reduce the dosage of other drugs by 50%, then adjust according to patient’s response.
Pediatric patients
Safety and efficacy not established.
