I.M. injection sites





When selecting the best site for a child’s I.M. injection, consider his age, weight, and muscle development; the amount of subcutaneous fat over the injection site; the type of drug you’re administering; and the drug’s absorption rate. These guidelines may assist you in making a selection.


Vastus lateralis


Appropriate age



  • Infants


  • Toddlers


Needle size and length



  • Infants under 4 months: 23G to 25G, 5/8″ needle


  • Infants over 4 months and toddlers: 22G to 25G, 1″ needle


Recommended maximum amount



  • Give 1 ml or less to infants.


  • Give 2 ml or less to toddlers.


Special considerations



  • The vastus lateralis is a large, well-developed muscle with few major blood vessels or nerves.


  • The rectus femoris muscle should be avoided when using this injection site
