Therapeutic Actions
A preparation from soybean or safflower oil that provides neutral triglycerides, mostly unsaturated fatty acids; these are used as a source of energy, causing an increase in heat production, decrease in respiratory quotient, and increase in oxygen consumption.
Indications
Source of calories and essential fatty acids for patients requiring parenteral nutrition for extended periods
Essential fatty acid deficiency
Contraindications and Cautions
Contraindicated with disturbance of normal fat metabolism (hyperlipemia, lipoid nephrosis, acute pancreatitis), allergy to eggs.
Use cautiously with severe liver damage, pulmonary disease, anemia, blood coagulation disorders, pregnancy, jaundiced or premature infants.
Available Forms

Dosages
Adults
Parenteral nutrition: Should not constitute more than 60% of total calorie intake. 10%: Infuse IV at 1 mL/min for the first 15–30 min; may be increased to 2 mL/min. Infuse only 500 mL the first day, and increase the following day. Do not exceed 2.5 g/kg/day. 20%: Infuse at 0.5 mL/min for the first 15–30 min; infuse only 250 mL Liposyn II or 500 mL Intralipid the first day, and increase the following day. Do not exceed 3 g/kg/day. 30%: Infuse at 1 mL/min (0.1 g fat/min) for first 15–30 min; do not exceed 2.5 g/kg/day.
Fatty acid deficiency: Supply 8%–10% of the caloric intake by IV fat emulsion.
Pediatric patients
Parenteral nutrition: Should not constitute more than 60% of total calorie intake. 10%: Initial IV infusion rate is 0.1 mL/min for the first 10–15 min. 20%: Initial infusion rate is 0.05 mL/min for the first 10–15 min. If no untoward reactions occur, increase rate to 1 g/kg in 4 hr. Do not exceed 3 g/kg/day. 30%: Initial infusion, 0.1 mL/min (0.01 g fat/min) for first 10–15 min; do not exceed 3 g/kg/day.Stay updated, free articles. Join our Telegram channel
