Therapeutic Actions
Mechanism of action is not clear. Has properties of all antiarrhythmic classes; affects action potential of cardiac cells to alter conduction.
Indications
To reduce risk of cardiovascular hospitalization in patients with paroxysmal or persistent atrial fibrillation or atrial flutter with a recent episode of either and associated cardiovascular risk factors (age older than 70 yr, hypertension, diabetes, prior stroke, left atrial diameter 50 mm or more, left ventricular ejection factor less than 40%) who are in sinus rhythm or will be cardioverted
Contraindications and Cautions
Contraindicated with hypersensitivity to components of drug, class IV HF or recent acute failure requiring hospitalization or referral to a specialized HF clinic, second- or third-degree heart block, sick sinus syndrome, pulse rate less than 50, concomitant use of drugs or herbs that prolong QT interval, prolonged QT interval, severe hepatic impairment, concurrent use of CYP3A inhibitors, pregnancy, lactation, permanent atrial fibrillation.
Use cautiously with mild to moderate hepatic impairment, hypokalemia, hypomagnesemia, mild to moderate HF.
