Therapeutic Actions
Bactericidal: Inhibits synthesis of bacterial cell wall, causing cell death.
Indications

Skin and skin structure infections caused by S. aureus, Staphylococcus epidermidis, streptococci, E. coli, Proteus mirabilis, Klebsiella, Bacteroides, Clostridium, Peptococcus, Peptostreptococcus

UTIs caused by E. coli, P. mirabilis, Klebsiella, Morganella morganii, Proteus rettgeri, Proteus vulgaris, Providencia

Uncomplicated gonorrhea caused by Neisseria gonorrhoeae

Intra-abdominal infections caused by E. coli, Klebsiella, Bacteroides, Clostridium
-
Gynecologic infections caused by E. coli, N. gonorrhoeae, Bacteroides, Clostridium, Peptococcus, Peptostreptococcus, group B streptococci

Septicemia caused by S. pneumoniae, S. aureus, E. coli, Klebsiella, Bacteroides

Contraindications and Cautions

Contraindicated with allergy to cephalosporins or penicillins.

Use cautiously with renal failure, lactation, pregnancy.

Powder for injection—1, 2 g; injection—1 g/50 mL, 2 g/50 mL in D5W
Dosages
Adults
1–2 g IM or IV every 6–8 hr, depending on the severity of the infection.

Uncomplicated gonorrhea: 2 g IM with 1 g oral probenecid.

Uncomplicated lower respiratory infections, UTIs, skin infections: 1 g every 6–8 hr IV.

Moderate to severe infections: 1 g every 4 hr IV to 2 g every 6–8 hr IV.

Severe infections: 2 g every 4 hr IV or 3 g every 6 hr IV.

Perioperative prophylaxis: 2 g IV or IM 30–60 min prior to initial incision and every 6 hr for 24 hr after surgery.

Cesarean section: 2 g IV as soon as the umbilical cord is clamped, followed by 2 g IM or IV at 4 and 8 hr, then every 6 hr for up to 24 hr.
