Direct activated thrombin inhibitor
Therapeutic Actions
Inhibits free and clot-bound activated thrombin. Thrombin enables the conversion of fibrinogen into fibrin fibers during the clotting cascade; inhibiting this process prevents clot development. Apixaban had no direct effect on platelet aggregation but does inhibit platelet aggregation induced by thrombin.
Indications
Reduction of the risk of stroke and systemic embolism in patients with nonvalvular atrial fibrillation.
Contraindications and Cautions
Contraindicated with history of serious hypersensitivity reactions to apixaban, active pathological bleeding, pregnancy, lactation, hepatic impairment, presence of prosthetic heart valves.
Use cautiously with invasive procedures, surgery.

Tablets—2.5, 5 mg
Dosages
Adults
5 mg PO bid. For patients 80 yr or older, body weight 60 kg or less, serum creatinine 1.5 mg/dL or more, 2.5 mg PO bid.
Pediatric patients
Safety and efficacy not established.
