The first drugs used to treat depression were amphetamines. They activated the central nervous system and disguised the client’s depressed behavior, but had the potential to be abused. Next came the first generation of antidepressants, which included the monoamine oxidase inhibitors (MAOIs), and the tricyclic antidepressants (TCAs). The second generation of antidepressants include the selective serotonin reuptake inhibitors (SSRIs) and other miscellaneous drugs. The antidepressants have a number of effects on the body besides relieving the signs of depression. They help to regulate sleep and appetite, enhance mental alertness and lessen suicidal thoughts.

ANTIDEPRESSANTS CLIENT TEACHING
For All Antidepressants
Drug can take up to 3 to 4 weeks to relieve depression.
Take drug even if you feel symptom free.
TCAs
Take drug as prescribed.
Get up slowly when lying down and sitting up to avoid orthostatic hypotension.
Tell physician if you develop fast heart rate, constipation, urinary retention, blurred vision, photophobia, or dry mouth.
No activities requiring concentration until response to medication is known
Do not breast-feed.
No OTC drugs until discussing with physician
Stay out of the sun.
MAOIs
Avoid all foods and beverages containing tyramine (aged meats and cheeses, yeast, fava beans, red wines, smoked meats).
No OTC drugs until discussing with physician
Report headache immediately.
No caffeine
Change positions slowly.
Do not breast-feed.
Weigh yourself 2 to 3 times per week.
SSRIs
Report skin rash.
Do not breast-feed.
No activities requiring concentration until drug effects are known
Diabetics need to monitor blood glucose.
Miscellaneous Antidepressants
Do not breast-feed.
Drug must be taken at same time every day.
Drug cannot be stopped abruptly.
No activities requiring concentration until drug effects are known
No alcohol
