Therapeutic Actions
Normal blood protein; maintains plasma osmotic pressure and is important in maintaining normal blood volume.
Indications
Emergency treatment of shock due to burns, trauma, surgery, and infections (20% or 25%); 5% for use in nonemergency cases
Burns: Used to prevent hemoconcentration and water and protein losses in conjunction with adequate infusions of crystalloid. 20% or 25% used beyond 24 hr.
Hypoproteinemia in postoperative patients, patients with sepsis, or intensive care patients (20%, 25%)
Adult respiratory distress syndrome: Albumin 20% or 25% with a diuretic may be helpful
Cardiopulmonary bypass: Preoperative blood dilution with 20% or 25% albumin. 5% may be used as adjunct therapy.
Acute liver failure (20% or 25%)
Sequestration of protein-rich fluids (20%, 25%)
Erythrocyte resuspension: Albumin 20% or 25% may be added to the isotonic suspension of washed red cells immediately before transfusion
Acute nephrosis: Albumin 20% or 25% and loop diuretic may help to control edema repeated daily for 7–10 days
Renal dialysis: Albumin 20% or 25% may be useful in treatment of shock and hypotension. 5% solution may be used as adjunct in hemodialysis.
Hyperbilirubinemia and erythroblastosis fetalis: Adjunct in exchange transfusions
Neonatal hemolytic disease: 20%–25% solution
Contraindications and Cautions
Contraindicated with allergy to albumin; severe anemia, cardiac failure, normal or increased intravascular volume, current use of cardiopulmonary bypass.
Use cautiously with hepatic or renal failure.
